Depression and anxiety are found in every society in the world, debunking old theories that only Westerners get depressed.

These new findings come from the world’s most comprehensive study of anxiety and depression research to date, published by researchers at The University of Queensland.

In two separate studies of anxiety disorders and major depressive disorder (that is, clinical depression) study authors found that surveys of clinical anxiety and depression have been conducted across 91 countries, involving more than 480,000 people.

sleeping womanTired? Scientists have discovered another possible benefit of a night of restful and uninterrupted sleep.

According to a new study led by researchers at the Johns Hopkins Bloomberg School of Public Health fragmented or interrupted sleep could predict future placement in a nursing home or assisted living facility. The study is featured in the July 2012 issue of the Journal of the American Geriatrics Society and outlines the association between objectively measured sleep and subsequent institutionalization among older women.

“Sleep disturbances are common in older people,” said Adam Spira, PhD, lead author of the study and an assistant professor with the Bloomberg School’s Department of Mental Health. “Our results show that in community-dwelling older women, more fragmented sleep is associated with a greater risk of being placed in a nursing home or in a personal care home. We found that, compared to women with the least fragmented sleep, those who spent the most time awake after first falling asleep had about 3 times the odds of placement in a nursing home. Individuals with the lowest sleep efficiency—those who spent the smallest proportion of their time in bed actually sleeping—also had about 3 times the odds of nursing home placement.” The authors found similar patterns of associations between disturbed sleep and placement in personal care homes, such as assisted-living facilities. Sleep duration per se did not predict placement in either of these settings.

According to the Centers for Disease Control and Prevention, insufficient sleep is associated with a number of chronic diseases and conditions—such as diabetes, cardiovascular disease, obesity and depression. In addition, insufficient sleep is associated with the onset of many diseases and is responsible for motor vehicle and machinery-related crashes. glass-of-wineDrinking even “moderate” amounts of alcohol increases dementia risk.

Light to moderate alcohol consumption has generally been considered to have some health benefits, including possibly reducing risk of cognitive decline. However, two studies reported today at the Alzheimer’s Association International Conference® 2012 (AAIC®2012) in Vancouver suggest that moderate alcohol use in late-life, heavier use earlier in life, and “binge” drinking in late-life increase risk of cognitive decline.

“The many dangers of misuse of alcohol, and some of its possible benefits, have been widely reported, and there needs to be further clarification by the scientific community,” said William Thies, PhD, Alzheimer’s Association® chief medical and scientific officer. “Certainly no one should start drinking in order to reduce Alzheimer’s risk, as these two new reports attest.”

“We need to know more about what factors actually raise and lower risk for cognitive decline and Alzheimer’s disease. To do that, we need longer term studies in larger and more diverse populations, and we need more research funding to make that happen. We have learned incredible amounts about heart disease and stroke risk from long-term research like the Framingham Study – we have solidly proven lifestyle risk factors that people can act on every day. Specific types of “mindfulness practices” including Zen meditation have research-proven benefits for patients with certain physical and mental health problems, according to a report in the July Journal of Psychiatric Practice. The journal is published by Lippincott Williams & Wilkins,a part of Wolters Kluwer Health.

“An extensive review of therapies that include meditation as a key component—referred to as mindfulness-based practices—shows convincing evidence that such interventions are effective in the treatment of psychiatric symptoms and pain, when used in combination with more conventional therapies,” according to Dr William R. Marchand of the George E. Wahlen Veterans Affairs Medical Center and the University of Utah in Salt Lake City.
Mindfulness Techniques Show Health Benefits

Dr Marchand reviewed published studies evaluating the health benefits of mindfulness-based practices. Mindfulness has been described as “the practice of learning to focus attention on moment-by-moment experience with an attitude of curiosity, openness, and acceptance.” Put another way,
“Practicing mindfulness is simply experiencing the present moment, without trying to change anything.”

The review focused on three techniques:

Zen meditation, a Buddhist spiritual practice that involves the practice of developing mindfulness by meditation, typically focusing on awareness of breathing patterns.

Mindfulness-based stress reduction (MBSR), a secular method of using Buddhist mindfulness, combining meditation with elements of yoga and education about stress and coping strategies.

Mindfulness-based cognitive therapy (MBCT), which combines MBSR with principles of cognitive therapy (for example, recognizing and disengaging from negative thoughts) to prevent relapse of depression.

Dr Marchand found evidence that MBSR and MBCT have “broad-spectrum” effects against depression and anxiety and can also decrease general psychological distress. Based on the evidence, MBCT can be “strongly recommended” as an addition to conventional treatments (adjunctive treatment) for unipolar depression. Both MBSR and MBCT were effective adjunctive treatments for anxiety.

Research data also supported the effectiveness of MBSR to help reduce stress and promote general psychological health in patients with various medical and/or psychiatric illnesses. On its own, MBSR was helpful in managing stress and promoting general psychological health in healthy people.

There was also evidence that Zen meditation and MBSR were useful adjunctive treatments for pain management.

How do these practices work to affect mental and physical health? Dr Marchand discusses recent research showing the impact of mindfulness practices on brain function and structure, which may in part account for their psychological benefits. “These mindfulness practices show considerable promise and the available evidence indicates their use is currently warranted in a variety of clinical situations,” he concludes.
The article includes some proposed evidence-based guidelines for incorporating mindfulness-based practices into health care. So far there’s little evidence on which patients are most likely to benefit, but Dr Marchand suggests that patient preferences and enthusiasm are a good guide. He comments, “The most important considerations may be desire to try a mindfulness-based practice and willingness to engage in the regular practice of seated meditation.”
